Certification Request Form

Please use this document to request certification for an apprentice who has completed their Improve Proficiency Apprenticeship in England and Wales. Payment for certification (the standard registration/certification fee is £50, this includes VAT where applicable) should have been made when the apprentice was initially registered with Improve Ltd. i.e. within eight weeks of commencing his/her apprenticeship programme. If certification is validated, certificates will be sent to the claiming organisation within twenty working days of receipt of the certification request.
